We have identified multiple trends within our flight and maintenance operations that have reduced safety risk by using WBAT.

Please explain how you mitigated the issue or problem:

The Event Review Committee (ERC) develops many mitigations and corrective actions within a “just culture” environment. The largest trend reduction was in the area of air proximities. Through the reports, we were able to identify where the events were occurring and were able to change procedures to mitigate the risks.

How do you use Fuzion Safety’s web-based platform for SMSs? Through our electronic web and smart device applications for the management of our Company-wide SMS program, reporting and tracking, record keeping, auditing, action planning, and risk mitigations. We plan on using the program for the pending full Part 135 SMS implementation requirements.

What do you consider the most useful part of the SMS platform? The ASAP module is the most useful and has the most value for our interactions with the FAA. This gives us confidence in transparency when working through operational issues with them. Internally, the hazard reporting has the most value in ensuring we are proactive in finding our program’s and operational latent risks.

Please identify a recent safety issue or problem you identified in your data: During a review of our Standard Operating Procedures through Fuzion Safety’s SMS reporting system, we found an SOP that was not clear and could have been misunderstood by flight crews that could have led to an incident. After review and discussion within our Safety Committee, we updated the SOP and are confident that we proactively averted any possible associated issue.

Please explain how you mitigated the issue or problem: We updated the Standard Operating Procedures, advised pilots through a change notice, and added a procedure to our pilot training program to maximize our risk-reduction process.

This edition of Fuzion Safety’s Subscriber Spotlight features responses from Aaron Fettig, Aviation Safety Specialist at University of North Dakota (UND), a four-year college with a strong liberal arts foundation. Located in Grand Forks, ND, UND is home to the John D. Odegard School of Aerospace Sciences, which offers undergraduate and graduate degrees in a variety of aerospace disciplines, including Aviation, Air Traffic Management, Aviation Safety and Operations, Unmanned Aircraft Systems Operations, Atmospheric Sciences and Space Studies. UND’s aviation program is accredited by the Aviation Accreditation Board International (AABI) and offers FAA-approved curricula for airplane and helicopter flight training. UND employs roughly 300 pilot employees and conducts flight training for approximately 1,500 flight students.

How do you use Fuzion Safety’s web-based platform for SMSs? UND uses Fuzion Safety’s SMS platform to collect and analyze safety reports as part of our Aviation Safety Action Program (ASAP).

What do you consider the most useful part of the SMS platform? Fuzion Safety’s SMS platform’s forms and taxonomy are fully customizable, allowing us to capture data valuable to UND and access that data efficiently to analyze safety trends.

Please identify a recent safety issue or problem you identified in your data: UND recently noticed an increase in reports of traffic conflicts at non-towered airports in the local area and was able to identify specific problem areas based on tracking recommended by our Event Review Committee (ERC).

Please explain how you mitigated the issue or problem: With feedback from our ASAP reports, a change to UND’s Safety Policies and Procedures was made to standardize certain procedures in the local area to reduce the number of traffic conflicts, specifically in cases when IFR aircraft are conducting instrument approaches into local airports with heavy VFR traffic.

This edition of Fuzion Safety’s Subscriber Spotlight features responses from Tiffany Whilden, Director of Safety at Ryan Air, a part 135 operation with 180 employees serving over 72 communities from 8 hubs in the state of Alaska. Ryan Air operates 21 aircraft, including Casa 212s, Pilatus PC-12s, Saab 340As, Cessna207s, and 208EXs. Ryan Air’s mission is: “To raise the quality of life of the people we serve” in a state where 82% of communities depend on air transportation to meet their needs. 

How do you use Fuzion Safety’s web-based platform for SMSs? Ryan Air uses the platform for some functions of each component in the SMS. Employees across the state have access to report concerns, areas of improvement, or event occurrences whether online or offline. Our Flight Crew have access to the platform from their EFBs. The mobile app makes submitting reports a lot easier for all employees. 

Most useful part of the SMS platform: Safety reports are centralized to one location. Queries are used to pull data. Adding pre- and post-risk factors to reports that come in. Tracking and trending. All risk acceptance decision-makers have access to the same information when processing reports. 

Please identify a recent safety issue or problem you identified in your data: We primarily move cargo and have been increasingly moving passengers (in the 207s and 208s). A pilot suggested that we incorporate a small mirror in the flight deck to increase safety and security so that the flight crew can always have an eye on what’s happening in the rear of the aircraft. 

Please explain how you mitigated the issue or problem: A small 1.5”x1.5” mirror with a 2”x2” plastic L bracket and Velcro was assembled by flight ops personnel. Maintenance conducted an assessment on the mirror to ensure that there wouldn’t be any interference with the flight instruments. After all the interfacing departments made the approval, flight ops secured the mirror in the flight deck for an additional layer of safety and security for our flight crew.
